Accepting Referrals

We are pleased to accept referrals to Bloom Psychiatry, a new and growing outpatient psychiatry clinic in Calgary. We offer both private and Alberta Health-covered services with a focus on trauma-informed, evidence-based care.

Thank you for considering Bloom Psychiatry for your patient's psychiatric care. We value collaborative relationships with referring providers and strive to provide timely, thorough consultation services.

Referral Process

1. Complete the referral form (available for download below).

2. Include relevant documentation such as previous assessments, medication history, filling out each field in detail.

3. Please fax referrals to (877) 360-7332, or email to referrals@bloompsychiatry.ca

 

4. If you have questions please email us at info@bloompsychiatry.ca or call us at (403) 300-1562.

5. Our team will review the referral within 5 business days

6. We will contact your patient directly to schedule their appointment

7. Following the assessment, a consultation letter will be sent to you with our findings and recommendations.

Referral Form

Please download our referral form using the button below. The form can be filled, then printed and faxed, or completed, scanned, and emailed to our secure referral address.

Referral Criteria

We currently accept referrals for adult patients (18+) with:

  • ADHD (assessment, diagnosis, and treatment recommendations)

  • PTSD and trauma-related conditions

  • Mood and anxiety disorders

  • Somatic symptom disorders and functional neurological disorders

  • Patients requiring diagnostic clarification

 

Important Requirements

  • Patient must have an active primary care provider who will continue to manage ongoing care

  • Patient must be willing to attend in-person assessment in Calgary (some follow-up may be available virtually)

  • For Alberta Health covered services, a complete referral form is required

We Are Not Able to Accept Referrals For:

  • Acute psychosis or mania requiring urgent intervention

  • Active substance use disorders as the primary concern

  • Patients with ongoing psychiatric care already in place

  • Patients with a significant history of violence or forensic involvement
